Please print & turn in on the first day of Band Camp
The Dekalb Medical Form is required for all students each school year.
We have minimized as much as possible the collection of duplicate information from form to form, but please complete every page since forms may be separated and filed in different locations.
Please print and fill out legibly for each student. Turn in to Dr. Fairchild by July 15. We are required to have physical copies from each student.
Remember to have your student complete his/her portion of the Field Trip consent.
Please include front and back of medical insurance card on Consent for Medical Treatment.
