DISTRICT BAND FORMS DEADLINE

Hello and congrats to you/your student for being selected to join the district honor band! We want to ensure all families are aware of the requirements for participation this year. All students must: 1) Email a signed copy of the Commitment Form (attached) to lakesidebandassociation@gmail.com: 2) Complete the Medical Release Form: https://docs.google.com/forms/d/e/1FAIpQLSeBL49bT0kiou83tCvDpCsXxI9iYCWulIrCe_eUV5wPIaWmQw/viewform 3) Pay the $25 participation fee. Payment details are listed below. The commitment form and payment are due by January 9, 2026. If these items are not submitted by the deadline, we will assume your student will not be participating. Additional information from the district is can be found on the first page of the Medical Release Form.
